1. Purpose This Agreement sets forth the terms and conditions under which the Participant will take part in The SHAPE Campaign, Season 5 (the “Project”), produced by The Raquez Rouse Co. Photography & Media, LLC (“Company”). SHAPE (Simply Human Advocating Positivity Everywhere) is a creative, immersive experience that includes photography, video, and promotional content. 2. Effective Date & Duration This Agreement becomes effective immediately upon acceptance by the Participant (via electronic checkbox or signature) and remains in effect indefinitely. The scheduled production period for Season 5 is January 29 – February 3, 2026. However, this Agreement continues beyond those dates to cover ongoing obligations, including but not limited to: Payment and processing terms Participation in post-production activities Media rights granted to the Company Liability and damages related to lodging or participation 3. Participant Experience The Participant will receive: Each Participant will receive three (3) professionally edited images from each photoshoot chapter (“Naked,” “Iconic,” and “Indigo”), totaling nine (9) images included in their package. If the Participant wishes to purchase additional edited images beyond the included set, they may do so at the rate of $25 per image. Payment for additional images is due at the time of selection. Styling, makeup, and wardrobe direction Participation in SHAPE talk show filming Exclusive formal welcome dinner with cast + crew Inclusion in promotional and behind-the-scenes content 4. Lodging & Travel Lodging Package Participants: Hotel accommodations will be provided for 4 nights (Thursday – Sunday), based on shared double-occupancy. Certain participants may be required to stay through Monday night (5 nights total) due to production scheduling. This additional night will be covered by the Company at no extra charge to the Participant. All participants must check out no later than Tuesday, February 3, 2026. Travel arrangements to and from the event location are the sole responsibility of the Participant. 5. Payment Terms Processing Fee Notice: The processing fee applies to all transactions regardless of payment method (credit/debit, Apple Pay, Google Pay, Klarna, PayPal, etc.). Payments are non-refundable once submitted. Flexible payment options are available through integrated processors (e.g., Klarna, Afterpay), subject to their respective terms. Option A: Local Access Experience Base Price: $725 Processing Fee: $43 Total Due: $768 Option B: Signature Access Experience (with Lodging) Base Price: $999 Processing Fee: $59 Total Due: $1,059 Option C: (Single Occupancy Lodging-Private Room): Signature Access Base Price: $1650 Processing Fee: $85 Total Due: $1,735 6. Conduct & Respectful Participation The Participant agrees to maintain professionalism and respect toward all cast, crew, and Company representatives. Specifically, the Participant agrees to refrain from: Harassment of any kind (verbal, physical, or online) Sexual harassment or inappropriate conduct Threats, intimidation, or violence Any behavior that disrupts the production environment Violation of this clause may result in removal from the Project without refund. 7. Media Release The Participant grants The Raquez Rouse Co. Photography & Media, LLC full rights to use their image, likeness, voice, and story in photography, video, audio, and promotional materials in perpetuity, across all media formats, worldwide, without additional compensation. 8. Lodging Responsibility & Damages The Company will book hotel rooms under its corporate account. The Participant acknowledges and agrees: Any damages, theft, or excessive cleaning fees charged by the hotel due to the Participant’s actions will be the financial responsibility of the Participant(s). In the case of double-occupancy, both occupants will be held equally responsible, regardless of who caused the damage. Liability for damages is capped at $500 per Participant per incident. 9. Governing Law & Jurisdiction This Agreement shall be governed by the laws of the State of North Carolina. Any disputes arising from participation in SHAPE Season 5 will be resolved exclusively in the courts of Wake County, NC. 10.
